In this session, we will first describe New York State’s (NYS) vision for the delivery of health-related social need (HRSN) services through the NYS 1115 Medicaid waiver. This session will provide a model for collaboration between health information exchanges (HIEs) and social care networks (SCN) to transform the delivery of HRSN services through planned interoperable exchange. This session will also provide best practices for codifying and sharing HRSN data from systems with varying levels of technical capabilities and in alignment with Gravity Project standards. In working together, HIEs and SCNs can achieve better outcomes for our neighbors and across the state through collaboration, development of standards, and transformative practices.
2. Discuss pain points associated with ingesting data from many systems and stakeholders with varying degrees of technical capacity and share lessons learned from their work with Gravity Project to develop new FHIR-based standards for the exchange of HRSN data across the network of HIEs in NYS.
