Session Objectives
1. Highlight work with TogetherNow, a leading SCN in the Finger Lakes Region that has been building a human services network and supporting platform since 2019, to design and prototype new ways to implement and optimize bi-directional exchange of HRSN data.
2. Discuss pain points associated with ingesting data from many systems and stakeholders with varying degrees of technical capacity and share lessons learned from their work with Gravity Project to develop new FHIR-based standards for the exchange of HRSN data across the network of HIEs in NYS.
