Description
This general education presentation, facilitated by the Diabetes MATCH Initiative (Mobilizing Access Through Capacity Building and Health Equity), will explore the collaborative potential of various health care entities—such as Health Information Exchanges, Community Information Exchanges®, and Community Care Hubs. The focus will be on supporting electronic information exchange between clinical and community-based organizations, particularly for referrals to the National Diabetes Prevention Program (National DPP) lifestyle change program (LCP) and diabetes self-management education support services (DSMES). Emphasizing health equity and addressing social determinants of health (SDOH), these entities are crucial partners in improving health care outcomes and ensuring equitable access to services. Attendees will gain insights into leveraging these partnerships to bridge clinical and community settings, fostering a holistic approach to health care delivery.
Session Objectives
1. Illustrate the importance of intermediaries in building community-clinical linkages (i.e., Health Information Exchanges, Community Information Exchanges, and Community Care Hubs) in addressing health-related social needs (HRSN) and promoting health equity and supporting referrals to the National DPP lifestyle intervention and DSMES.
2. Discuss common misconceptions and emphasize their complementary roles and the value of collaboration.
3. Provide actionable strategies and case studies to empower attendees in fostering meaningful partnerships between clinical and community-based organizations, with a focus on tangible outcomes for improving HRSN and enhancing overall population health.
