This panel will discuss how DC’s unique cross-sector partnerships enable the growth and success of the DC health information exchange (HIE), particularly its Population Health Analytics tool. While the CRISP DC HIE serves as the infrastructure that pulls data together, DC’s Medicaid agency has convened and nurtured partnerships across the health system, prioritized the use of HIE to support value-based and other whole-person care initiatives, and set the stage for the use of a common tool for quality measure performance management across providers and payers. This panel will highlight the unique and complex partnership across DC’s primary care association, FQHCs, CRISP DC, and the Medicaid agency to support an improved and coordinated level of care. This flexible model achieves stakeholders' specific and collective objectives through a responsive approach poised to address and plan for future HIE opportunities including predictive modeling, value-based payments, and risk stratification.
