The CMS Innovation Center (CMMI) has partnered with Health Information Exchanges (HIEs) and other data aggregators, as well as payers, to provide more accurate and complete data to providers to support their performance in value-based care models. Data aggregators receive CMS data and incorporate them with other claims and clinical data to deliver focused reports that target key model outcomes. Aggregators also provide support to providers in integrating tools into their workflows and interpreting and acting on data insights.
This session will present CMMI’s experience partnering with HIEs and other payers, including leveraging local stakeholders’ strong relationships with providers and the learning and support systems in place, to provide data insights to support value-based care transformation. Furthermore, this session will highlight how engaging the partnership with the HIE community aligns with CMMI’s broader future vision and goals for data use. Further discussion will focus on the impact of data aggregation use cases on practice performance in the model and the enhancements that have been provided based on practice and payer outcomes and feedback. This interactive session will provide an opportunity to hear directly from CMMI and aggregators they have partnered with to further value-based care outcomes.
