Description
Community Health Workers (CHWs) and community-based care coordination have been at the forefront of many major policy changes, briefs, and programming discussions – particularly since the COVID crisis. Health Impact Ohio has sought to support the ongoing evolution of the role of CHW, how they are placed to meet the community needs, and the sustainability – of funding, policy, and data transparency. In this presentation, HIO describes the CHW-related efforts and how the team seeks to evolve the work.
Session Objectives
1. Understand CHW Workforce and Training across the state, including CHW Certification programs, CHW employers and emerging trends.
2. Describe the Pathways Community HUB Model and its use of CHWs in Ohio, including a policy background regarding how HUBs became (and are still the only) the current way to receive Medicaid MCO reimbursement for CHWs in Ohio.
3. Describe limitations to the current operating CHW reimbursement in Ohio and recent evolutions in policy and reimbursement for CHWs in Ohio, including the recent CMS rule for Medicare reimbursement for CHW services, the emerging Community Health Equity Collaborative, and work underway from the newly-established Center for CHW Excellence.
