Unmet health related social needs are driving poor health outcomes among vulnerable communities throughout the U.S. Multi-sector and multi-stakeholder state collaboratives are coming together to address inequities at a systems level, and to apply policy and evidence to improve the structural determinants of health.
During this 90-minute session we will hear from presenters about using data-enabled and community-centric approaches to address health disparities and provide informed comprehensive care within community settings. We will also discuss strategies for securing state funding to sustain these initiatives. The event will feature presentations from collaborators in California, Michigan, Texas, Washington, and Wisconsin.
