Description
In the same spirit as the SHIEC and NRHI merger that formed Civitas, HEALTHeLINK, the health information exchange (HIE) servicing Western New York, merged with Population Health Collaborative, a Regional Health Improvement Collaborative in 2022 to establish a platform for multi-stakeholder, cross-sector collaboratives dedicated to reducing health disparities and improve health equity across its local community. Our presentation will highlight the history of the merger, the importance of the expansion of HEALTHeLINK’s mission to incorporate regional health improvement initiatives, the need for access to current, granular, demographically filterable data, the development of HEALTHeLINK’s comprehensive, publicly accessible community health dashboard, and how to translate HIE data into actionable strategies and collaboration to drive change in community health outcomes.
Session Objectives
1. Demonstrate the integration of clinical information exchange with regional health improvement activities.
2. Emphasize the importance of utilizing clinical information exchange data to report health disparities accurately.
3. Facilitate collaboration between physicians and community-based organizations through HIE to address and reduce health disparities.
4. Implement the use of HIE data to support regional health improvement by developing a community dashboard.
