Civitas Networks for Health, Gravity Project, and New York eHealth Collaborative (NYeC) are excited to present insights and outcomes from New York’s statewide program to standardize the collection and exchange of Health-Related Social Needs Screening Data using Gravity data standards. Through this innovative program, NYeC is convening partnering with the six State Qualified Entities (Bronx RHIO, Healthix, HealtheConnections, Hixny, HEALTHeLINK, and Rochester RHIO) to develop a standard set of functional and implementation requirements for the ingestion, handling, and use of Social Determinants of Health (SDOH) data across NY's Statewide Health Information Network (SHIN-NY).
In this interactive panel presentation, we will share an overview of the program and the Gravity Project, provide valuable real-world implementation insights, key takeaways, and successful outcomes initiated as part of the Medicaid 1115 Waiver implemented by CMS and the NYS Department of Health, Office of Health Insurance Programs.
