Data alone is not enough to advance health equity or improve population health outcomes. As a Regional Health Improvement Collaborative, we help ensure data is actionable by utilizing several strategies such as 1) including residents and partner organizations in planning and each subsequent phase of a project; 2) utilizing trusted community networks to deliver health information and services; and 3) convening solution design tables inclusive of stakeholders with lived experience. This presentation will feature outcomes from a multi-stakeholder, youth mental health initiative and a network of barbershops and beauty salons with staff trained to administer Naloxone. These are just two examples of the many ways Common Ground Health helps turn data into action to improve health equity and community health in our region.
