Recent legislation and investment in clinical and social data linkages have encouraged communities across the country to invest time in exploring interoperable multisector data-sharing systems that support person-centered, holistic care. This presentation will explore the technical and data underpinnings of a community-led care coordination infrastructure, with a specific focus on the community information exchange (CIE) model. Presenters will provide guidance for communities on the requirements that must be made of technology partners at the outset to ensure community ownership and autonomy, with the overall goal of working towards health equity.
