The California Quality Collaborative (CQC) will share design principles to support the development of a health equity centered quality improvement (QI) program. The session’s content is supported by the California Quality Collaborative’s (CQC) experience implementing the Equity and Quality at Independent, Practices in Los Angeles County (EQuIP-LA) program. Principles in health equity centered QI: -Incorporate voices of diverse stakeholders during the program design. -Develop a shared definition of health equity to guide throughout program. -Utilize ongoing assessment to monitor the implementation and impact across health equity metrics. Identify QI opportunities: -Utilize stratification approaches to identify patients and communities’ impact by health inequities. -Leverage and incorporate perspective of stakeholders to define the QI opportunity and potential change interventions. Collection, Analysis, and Interpretation of REaL data: -Approaches to data stratification that aligns with the programs scale. -Tools to share stratified results with program stakeholders to guide QI interventions.
