Description
This brief presentation followed by a panel discussion will highlight the promising results that come from using community-facing technology and community outreach to improve individuals' health, particularly those from underserved populations including people who have disabilities and their families. The presenters will speak from their experience about using person-centered service delivery and technology to simplify the navigation of community-based services and ease the burden on health care workers. While highlighting several concepts outlined in the Gravity Project's Social Care Co-Design Final Report, the presenter and panelists will share real-world examples of bright spots and lessons learned.
