Description
The Community Information Exchange (CIE) is a community-led model for holistic care coordination that shifts how health and social service providers deliver person-centered care. As cross-sector partnerships and data sharing between health and social services providers become more prevalent, so too has discussion around the feasibility and geographic scope of information sharing and system interoperability in support of community needs. This session will provide an overview of the CIE model and showcase Cook County's initial efforts to plan a regionwide CIE to create an equitable, effective, and sustainable care coordination infrastructure.
