Join us for an engaging session that explores the foundational principles of a Community Information Exchange (CIE). We’ll discuss what a CIE is—and what it is not—while examining its role in fostering cross-sector collaboration, supporting community needs, and advancing a shared vision for person-centered care. This session will clarify how CIEs function, their community-driven approach, and the importance of shared language and identity used to define and protect the role of a CIE within a broader ecosystem. You will have the opportunity to learn from communities and states across the nation that have implemented the CIE model—sharing best practices, insights, and lessons learned.

Presentation Learning Objectives
1). Define and differentiate the unique, community-driven approach of the Community Information Exchange (CIE) model from other data-sharing care coordination models.
2). Explain the role of shared governance in supporting the sustainability and integrity of a CIE within a broader ecosystem.
3). Gain insight into the CIE vision for the future, drawing from unique approaches, best practices, challenges, and lessons learned from communities that have implemented the CIE model.
