This panel highlights how strategic data management solutions and robust exchange platforms enable cross-sector data sharing, bridging silos to drive health and community improvements. It emphasizes the development of a strong CIE infrastructure, effective data stewardship practices, and enhanced whole-person care coordination to foster impactful outcomes.
Presentation Learning Objectives
1) Understand the Role of Cross-Sector Data Organizations and Data Bridging in Addressing Health and Community Challenges. This objective will be met by discussing the importance of Health Information Exchanges (HIEs) and Community Information Exchanges (CIEs) in bridging gaps between clinical and social data.
