Description
Session showcases how states are leveraging integrated claims-clinical data ecosystems to address critical public health challenges. Invited panelists from Delaware, Virginia, and Maryland will discuss and demonstrate how linking claims data from APCDs with clinical data through HIEs has the potential to provide more precise geographic tracking, reveals outcomes not captured in traditional surveillance systems, and supports targeted public health interventions. The session will highlight successful applications including environmental health tracking and care coordination initiatives.
Presentation Learning Objectives
1). Identify specific public health applications where integrated APCD-HIE data provides unique value beyond traditional surveillance systems. This objective will be met through detailed case presentations from DHIN on environmental health tracking of PFAS exposure and additional examples from other participating organizations showing how integrated data reveals insights not captured in siloed systems.
2). Understand the technical, governance, and operational frameworks required to successfully implement and maintain integrated health data ecosystems. Panelists will share their implementation journeys, with vendor representatives providing insights on data integration approaches, governance models that enable appropriate data sharing while maintaining privacy, and operational considerations for sustainability. Practical examples and lessons learned will illustrate both challenges and solutions.
3). Evaluate strategies for leveraging integrated data to improve specific public health outcomes and reduce healthcare disparities in attendees' own communities. The presentation will include analyses of how integrated data has been used to identify geographic and demographic disparities in health outcomes, target interventions more precisely, and measure impact. Panelists will discuss methodologies for applying similar approaches to different public health challenges, providing attendees with actionable frameworks for implementation in their own contexts.
