Description

Gravity Project members and community partners will discuss three initiatives to support connection to, enrollment in, and engagement in core human and social service programs. Based on implementers’ and patients’ needs, Gravity Project standards are evolving to better support engagement in services toward improved outcomes. Initiatives include a national open social care program classification, intelligent referrals and interoperability to facilitate enrollment and engagement, and emerging standards for documenting and sharing enrollment status toward sustained engagement.

Presentation Learning Objectives
1). Describe how emerging Gravity Project standards are evolving to support enrollment and sustained engagement in human and social service programs.
2). Examine real-world initiatives leveraging national social care program classifications, intelligent referrals, and interoperability to improve connection to services.
3). Understand the importance of documenting and sharing enrollment status to enhance outcomes and continuity of care across health and social systems.
