Description
This session will provide an overview of Community Information Exchange (CIE) and Health Information Exchange (HIE) models, including their origins, evolution, and core constructs. Presenters will explore how aligning these infrastructures can support community-led, person-centered care coordination. Attendees will gain a deeper understanding of the value, benefits, and definitions of each model, and learn how the integration of health and social information creates new opportunities for delivering holistic, coordinated care.
