The Network focuses on eliminating traditional silos and improving access to services for community members with unmet social needs. Broad adoption of the network by healthcare providers and CBOs is expected to increase referral volume, connecting individuals with services they need and improving their overall health and wellbeing.

Presentation Learning Objectives
1) Demonstrate how to engage stakeholders in codesign process for healthcare and human services network development. Presenters will provide case study of how our organization, in partnership with a healthcare system co-led network development and established a framework for shared decision making.
2) Explain the value of a hub and spoke model for responding to social care referrals. Presenters will explain the evolution of the model for triaging social needs referrals as a Social Care Hub.
3) Identify essential components of information exchange for an effective, closed-loop social care referral. Presenters will describe the process for prioritizing essential components of a referral when information is exchanged among healthcare and social service providers.
