Description

This panel discussion will equip participants with key insights into building or enhancing partnerships to advance referrals and create a connected system of care. Attendees will explore strategies for leveraging resources such as staff and funding, defining partner roles and responsibilities, and developing a roadmap for strengthening community-clinical linkages. Through collaborative discussions, participants will uncover how strategic partnerships can shape the future state of integrated care networks.
