Primary Care Information Blueprint: A Framework for Action in Value-Based Care
Description
Two things are essential to value-based care success – strong primary care and efficient bi-directional information flows across multiple stakeholders. The AAFP worked with family physicians and other key stakeholders to develop the Primary Care Information Blueprint. It outlines essential data and information needs in VBC and underscores the critical role data intermediaries, like HIEs and HDUs, play in meeting those needs. Learn about the blueprint and hear from organizations modeling its framework in action.
Presentation Learning Objectives
1) Understand the range of data/information needs of primary care practices delivering value-based care.
a) Will be met by a review of AAFP’s Primary Care Information Blueprint policy brief and discussion of methodology behind the blueprint.
2) Identify market and regulatory forces impacting primary care information sharing so you can address and influence in your own communities.
a) Will be met by discussing the current market and regulatory ecosystem and translating headwinds and tailwinds relative to information sharing at a national and regional level.
3) Describe the opportunities and challenges faced by Health Information Exchanges (HIEs) and Health Data Utilities (HDUs) as they implement real-world, practical examples of the Blueprint in action to illustrate how you can address in your own region.
a) Will be met by HIE’s presenting their respective case studies that illustrate the tenets of primary care information blueprint in action. If selected for presentation, we will invite presentations from MyHealth, Arkansas SHARE, CRISP or others as identified.
