Description

Community Information Exchanges (CIEs) and Community-Based Organizations (CBOs) form a critical partnership to serve communities in desperate need of coordinated health and human care by integrating and exchanging data. Amidst reduced federal funding and increasing austerity, efficiency becomes paramount. This presentation explores how aligning the right use cases optimizes resource allocation, strengthens cross-sector collaboration, and enhances service delivery. By leveraging data-driven strategies, CIEs and CBOs can maximize impact despite financial constraints.

Presentation Learning Objectives
1) Understand the Strategic Role of CIEs and CBOs in Coordinated Care: Attendees will learn how Community Information Exchanges (CIEs) and Community-Based Organizations (CBOs) integrate health and social data to address social determinants of health and improve service coordination for vulnerable populations.
2) Analyze Case Studies for Scalable and Efficient Data Integration: Using real-world examples from Alameda County (CalAIM), Sacramento County, and King County (WA) attendees will explore best practices for building efficient, cost-effective data-sharing models in an era of reduced federal funding.
