Description
The CKD Workgroup of the Utah Diabetes Coalition is utilizing data from the Utah CHIE (Clinical Health Information Exchange) to estimate CKD prevalence and prompt primary care follow-up for undiagnosed individuals. Over 49% of these individuals are missed, creating a quality of care concern and a large downstream cost on the healthcare system. This may be a contributing factor for Utah having the highest kidney disease mortality among western states.5

Presentation Learning Objectives
1) Attendees understand how Health Information Exchange data can be utilized to identify and estimate prevalence of chronic diseases. By analyzing lab results and diagnosis HIEs can help to identify potential patients at risk, potential undiagnosed patients, and diagnosed patients.
2) Attendees see an example of how Health Information Exchanges can help raise awareness of potential patient issues for busy practitioners. The Utah Chronic Kidney Disease Collaborative is interested in an alert that would go to providers for high risk patients (those with diabetes and hypertension) that have positive tests for kidney disease but do not yet have a chronic kidney disease diagnosis.
3) Attendees realize how partnering across state and local health departments, QIOs, HIEs, health systems and non-profits, benefitting from the expertise of each organization, can lead to multi-faceted approaches to address complex health issues at various points in the continuum and progression of the disease from prevention to intervention.
