Description
The University of Texas Health Science Center (UTHealth) at Houston will present the evaluation framework for Health Equity Collective CARE Network, an innovative closed-loop referral platform to bridge health care and social service agencies in Greater Houston. The evaluation strategy assesses the reach, adoption, implementation, and the effectiveness of the closed-loop framework in improving social and health outcomes. By leveraging multiple data streams, from health care utilization information to patient-reported outcomes, UTHealth is able to measure the system’s impact on health equity, efficiency, and cost reduction.
