Description

This 45-minutes interactive session will explore real-world experiences in developing Community Information Exchanges (CIEs) from national, state, and local perspectives. Experts from Wyoming 211, Wyoming Department of Health, EMI Advisors, and Hello Better Healthcare will share lessons learned, success stories, and challenges in integrating community and clinical data to enhance care coordination. A brief presentation will set the stage, followed by a large group discussion to foster networking, collaboration, and actionable takeaways for attendees.

Presentation Learning Objectives
1) Attendees will explore the role of intermediaries in strengthening CIE infrastructure and gain an understanding of national policies, interoperability frameworks, and governance models that support CIE development.
2) Attendees will identify effective strategies for engagement and data integration including developing key metrics, lessons learned, and best practices from implementing a community-driven CIE.
3) Attendees will learn how partnerships with HIEs, other intermediaries, and CBOs enhance care coordination and cross-sector collaboration.
4) Attendees will be able to describe sustainable models for long-term CIE success and gain actionable insights to support long-term sustainability of CIEs.
