Bridging Data and Action: Cross-Sector Collaboration for Community Health
Description

A cross-sector panel will highlight promising practices in social care information exchange. Leaders from the Bronx RHIO, SOMOS Community Care, God's Love We Deliver, and Findhelp will discuss leveraging data, particularly social needs screening, referral, and intervention data, to improve health outcomes. Supported by New York's Section 1115 Waiver, these initiatives demonstrate how effective data sharing empowers providers and CBOs to act on insights and address social needs.

Presentation Learning Objectives
1) Examine strategies for fostering effective cross-sector collaborations between the health and social care sector that leverage standardized data collection and health information exchange infrastructure.
2) Demonstrate actionable insights on the significance of data exchange and information sharing in improving care coordination, addressing social determinants of health, and advancing health outcomes.
3) Recognize the critical role of CBO engagement in shaping collaborative initiatives and ensuring that solutions are responsive to community needs while building CBO capacity to participate.
