Description

Discover how authentic community engagement and partner capacity building in a Community Information Exchange (CIE) can transform the regional social care landscape and advance health equity. This session will cover key lessons learned from HealthierHere’s six-year journey co-designing, launching, and expanding a CIE serving organizations across Washington State, and poised to grow further. This session will also explore how CIE networks and technology enhance collaboration and data sharing, elevate community partner efforts, and enable more equitable, sustainable whole-person care coordination systems.

Presentation Learning Objectives
1) Attendees will be able to describe how approaches to engaging community and cross-sector stakeholders changes from CIE visioning and development through launch, implementation, and governance.
2) Attendees will be able to discuss how to develop readiness and capacity among community-based organizations to participate in a CIE effort effectively.
3) Attendees will understand how participation in a community co-designed CIE amplifies the work of small CBOs, enhancing their capability to serve community members effectively and efficiently.
