Unlocking Health: The Power of Community Care Hubs
Description
Community Care Hubs (CCHs) offer a promising solution for addressing health-related social needs and improving outcomes for vulnerable populations. This session explores the innovative work of Comagine Health and CommonSpirit Health in implementing diverse CCH models across the nation. Attendees will gain insights into building capacity, establishing a sustainable business case, and leveraging CCHs to drive meaningful change within their communities.
Presentation Learning Objectives
By the end of this session, participants will be:
1) Able to identify the core elements of a Community Care Hub. Speakers will share information from the Pathways Community Hub Institute, Health & Human Services, and the Partnership to Align Social Care to orient participants to core elements.
2) Familiar with two nationally recognized Community Care Hub Models and use cases for implementation. Presenters will highlight the evidence base for the Pathways Community Hub Institute Model and the Administration on Community Living led CCH model as well as orient participants to the factors that contributed to success in implementation in regions of focus as well as learnings from implementing the models across different geographic areas.
3) Able to identify outcomes that resulted from better care coordination through CCH led interventions. Presenters will highlight data on how these models addressed Health Related Social Needs and resulted in improved outcomes for specific populations of focus from the pilots conducted in these regions. This includes establishing the infrastructure to track services and demonstrate impact.
