This lightning talk will explore how the TogetherNow CIE is collaborating with their regional HIE to create a payor-agnostic data exchange model. 1) Why CIEs and HIEs Must Work Together: Attendees will learn how CIEs/HIEs complement each other and why their integration is crucial for improving care coordination and addressing social needs.
2) Making Data Flow: A Payor Agnostic Approach: This talk will highlight how a payor agnostic model supports seamless referral and patient data exchange, ensuring all healthcare providers, regardless of funding source, can access and use critical social care information.
