Description

Join us for a deep-dive into the reality of data integration and interoperability using real-world examples that illustrate various design approaches, use-cases, and data models from San Diego CIE and University of Missouri, St. Louis (UMSL) Community Innovation & Action Center. We will explore the challenges and solutions used for several data integration efforts and how the iterative trial and error process informed the development of the new “CIE Technology Toolkit” and “Technology Checklist.”

Presentation Learning Objectives
1). Explore several real-world data integrations: Community care hubs, from a hospital to a CIE via an integration with a referral platform, Health Plans using CIE to send closed-loop referrals, faith-based social services, and more
2). Understand key technology, data, and design variables that shape data integration and system interoperability
3). Walk away with a technology check list of Do's and Don'ts when selecting technology for an interoperable CIE
