In 2022, execution of an ambitious project began, four Vermont state agencies teamed up with a technology partner to implement a configurable technology platform which enabled agency teams to share data and speed care coordination and services. In Texas individuals experiencing homelessness are discharged to a shelter with little notice minimizing the shelter’s ability to assure appropriate support services. Managed Care Organizations have been working with the Texas Homeless Network (THN) to coordinate safe transitions.
In this presentation, a moderated panel from Vermont State public health leaders, THSA, C3HIE, Superior and THN will discuss how they implemented shared technologies, worked across State programs, integrated existing infrastructure and added automation to speed assistance to families in need of housing, healthcare and other resources. The panel will discuss how team (housing and health) coordination and governance can allow for proper care delivery; improved outcomes, reduced readmits, and lower costs, by targeting the use of finite resources..
