Description

We will share lessons from the field on how building interoperable data infrastructure improves local cross-sector care coordination to effectively address social determinants of health. During this panel session, we will describe benefits of interoperability between the San Diego 211 Community Information Exchange and the San Diego Health Connect HIE. We will provide technical and governance overviews, use case examples, consent management strategies, and impact of evolving regulations including statewide Medicaid reform. The panel will discuss ways in which both health and social needs data advances health equity and provides opportunities to identify and engage populations affected by disparities. Actionable data allows providers to close gaps in care and fine tune service offerings.
