Multiple agencies within the Commonwealth of Kentucky collaborated to provide case managers with access to the health records of children in out-of-home care through the Kentucky Health Information Data Sharing (KHIDS) project. The central goal of KHIDS was to put critical health information in the hands of the case managers working with out-of-home youth by connecting Kentucky’s child welfare information system (TWIST) to the state health information exchange (KHIE). By having access to the critical health data stored in KHIE and its additional services, case managers can have a more robust understanding of the child’s need and perform the appropriate case management and care coordination activities to provide safety, permanency, and wellbeing to children in out-of-home care.
