In recognizing community need for health transformation and health equity, Clinisync took on the opportunity to include Social Drivers of Health (SDOH) data into their core data set to evaluate health outcomes and quality of life. The need for interoperability in client level data allowed for the local relationship with Health Impact Ohio to evolve so that the data from Community Health Workers within the Central Ohio Pathways HUB could account for closed loop referrals to see a holistic approach to client and community. This collaboration allows for a cohesive approach to serving those most in-need within the State and better understand gaps within the current landscape for those most at-risk.

The Finger Lakes region of NYS has a longstanding history of identifying measures to advance population health through community-driven process with stakeholders from healthcare, local public health, academic research and employers. Over time, the region shifted from using publicly available data sets toward producing regional health measures using HIE data. Recognizing the complementary strengths of the two organizations, Rochester RHIO and Common Ground Health are piloting a new model of shared resources to expand upon our existing work. NYeC provided funding to define and generate new community health measures focused on behavioral health, maternal health, and SDoH. This initiative is a pilot for a larger effort to promote population health improvement, grounded in health equity, through collaborative efforts.
